What CPD credit forms are accepted for certification, and how do I submit them?

To be accepted for credit, a completed CPD form must include signatures from both the course instructor and the HCPSS CPD Liaison or Non-Public School Official.  An example of a signed CPD form can be viewed hereSigned CPD forms must be uploaded to your TEACH application under the Continuing Education section.

NOTE: Frontline transcripts are not accepted as evidence of CPD credits earned.
