Staff members who trying to get pregnant are encouraged to discuss individual circumstances with their health care provider and complete the ADA accommodation medical questionnaire and return the form within 15 days of completion via email to ADA_Coordinator@hcpss.org or fax to (443) 973-5598.
ADA determinations are made on a case-by-case basis once a request has been submitted to the ADA coordinator. Determinations are dependent upon many factors including medical condition, essential job functions, and the school system's ability to reasonably accommodate the disability.
