Your leave of absence will begin as of the first date that you were/will be absent. Please be sure to communicate this date to your physician so that they may take this date into consideration when determining the period of incapacity for your leave of absence.
The leave of absence start date on your request for leave of absence form must match the start date on your physician certification form.
